IMPORTANT INFORMATION FOR PROSPECTIVE YOUTH ATHLETE MEMBERS:
1. Coach may sign with permission of parent

2. Youth addresses must be their physical mailing address, not be the club address.

3. New or lapsed membership must submit copy of birth certificate or other ID showing Name and DOB. Scan and email to services@usatfnj.org or copy and mail to the USATF-NJ office address in the next column.

Please Note:  Membership cards are processed and mailed by the National office.  The NJ office does not send them out.
To receive your new number in advance, you can include an email address on the form or enclose a Stamped Self-Addressed Envelope with your mailed in application.  We will return your new number to you promptly.

DOB verifications (US birth certificate, US passport, US Naturalization certificate, US Green Card/Visa or NJ Drivers License) can be scanned and emailed to services@usatfnj.org or copied and mailed to the above address.
